Insert description here.Every individual who comes to us for assistance is unique. Our team at Health Connect America (HCA) understands the importance of personalized service. Our approach offers a fresh, innovative way for individuals to access to mental health services. We address the multiple needs of each individual, their family and the significant people in their lives. We provide face-to-face support, counseling, and care coordination with the individual, family and significant others, as well as behavioral, emotional and educational support. Ideally, the support and services will be provided in the client’s home or community. We have developed a unique approach to engaging individuals, and others involved in the treatment process, to identify problems and explore solutions. For the services that are given in one of our multiple offices, HCA will attempt to include the family or other significant people in the individuals’ life, as appropriate. We utilize the most current and clinically appropriate treatment available for children, adults, and families at risk with the ultimate goal of keeping them safe while empowering them. Our services are personalized to the needs of the individual and the family, coupled with Health Connect America’s clinically-proven strategies and needs assessments. For individuals with substance use issues, we treat the whole person, ensuring both mental health needs and substance abuse needs are addressed simultaneously. Our trauma-informed approach allows individuals to resolve past and present traumatic issues in a safe and structured way. After receiving services with Health Connect America, individuals: • Have mastered new coping mechanisms • Have additional natural supports • Connect to services in their communities • Move forward in their lives to be positive participating members of their families, relationships and communities Our staff initially perform an extensive assessment, followed by office sessions and/or community visits in the home (or school if appropriate). We utilize mental health counseling, support and service models that foster open interaction and honesty among our team members and individuals. Those who seek services through us work closely with our staff to clearly define treatment/service plans and goals with target dates for achievement. Health Connect America team members provide documentation and ongoing updates regarding each individual’s progress to referral sources as well as coordinating with other involved professionals, such as Primary Care Physicians. Services are flexible and meet the needs and schedules of the individual and family.

Referral / Admission

  • Admission criteria
    • Admission is based upon service recommendations of the comprehensive evaluation. Call us at the office for more specific information for admission criteria.
  • Referral process
    • Call us at the office to be screened or inquire about specific criteria. After a short screening is completed, a counselor immediately contacts the client and/or family and schedules a comprehensive evaluation. The time period from a referral being made and services beginning is typically between 24-72 hours.

    • Our Psychiatric Clinical Services provides our clients with a psychiatric assessment with medication management. This program supports and enhances the therapeutic services we already provide to our clients and their families. The primary goals of this program are to provide a team-based approach, including mental health and health care providers. Health Connect America supplies every aspect of the treatment program. Doing so ensures quality and timely community-based assessment and medication management. This unique program can reach our most rural areas via telemedicine. Our clients also have a case manager who follows clients as they transition through other programs at Health Connect America to ensure coordination of care. The case manager facilitates appropriate referrals for long-term follow-up and maintenance of the client’s behavioral health
